- A 72-year-old woman with a history of coronary artery disease presents 2 hours after the abrupt onset of confusion and left-sided weakness. A CT perfusion scan reveals a large region of hypoperfusion in the right middle cerebral artery territory and a small region of severe reduction in cerebral blood flow in the right basal ganglia. The CT angiogram reveals a proximal occlusion of the right middle cerebral artery. Blood pressure is 187/112. Which approach would you recommend?
